Health Maintenance & Patient Follow-up

Health Maintenance Alerts

This module allows users to set up and tailor their Health Maintenance Reminders based on recommendations for clinical practice on preventative interventions, best practice guidelines or clinical care paths for specific diseases and disorders.  The module supports alerts that are set up for the whole practice.  Individual providers within the practice are also able to personalize alerts and tailor them more specifically for their work.

Health Maintenance AlertsThe presence or absence of a finding within the patient's chart will switch the alert on or off.  Alerts can be set so they are conditional on age, gender, the presence or absence of any other clinical factor, such as values out of a specified range or any other Medcin finding.  Testing or examination intervals can be set.  In addition, where a patient is at risk for a disease but has not yet been diagnosed with that problem the alerts can be assigned specifically, so that a screening protocol can be set up to monitor the patient for their specific risk factors.  Alerts can also be grouped so that they will always be displayed together, such as HbA1c, Blood Pressure checks, Lipid Measurement, LDL levels, foot examinations and monofilament tests for diabetic patients.  The ability to set these alert Supercodes will help to ensure that all aspects of prospective data collection for Clinical Quality Measurement are routinely gathered.

Follow-up Recalls

Follow-up & RecallsMedcomSoft Record's comprehensive patient-recall system enables practitioners to create multiple patient recalls based on desired intervals or required tasks (follow-up visit, lab testing, vaccination, or other procedures). Recalls can be quickly linked to specific encounters or conditions to ensure rapid review of outcomes and the optimization of patient care. Patient recalls can be filtered by date in advance to optimize the provider's appointment schedule and to ensure that important follow-ups are not missed.
